Study of Quality Control in Central Sterile Supply Department of a Tertiary Care Teaching Hospital

Hospital acquired infection or “nosocomial infection” adversely affects both patients and hospitals. Impact of nosocomial infections ranges from increased length of hospital stay, emotional stress, disability, death of the patients as well as increased hospital cost for the patients and providers. Studies in India have reported nosocomial infection rates from 8% to 58 %. To combat these infections, hospital needs effective methods of disinfection and sterilization which has nowadays been centralized into a single department called Central Sterile Supply Department. In our study Outcome was measures by two parameters: a) User Satisfaction b) Microbial count It was found that in user satisfaction 70% of user end staff are satisfied with the services provided to them. In microbial count of different items sterilized showed that 89 % samples are sterile whereas 11% samples show growth of three different organisms e.g. Coagulase-negative staphylococci, Klebsilla and Act.   There are perspectives to cure breast cancer, and the main strategy in that way is the exact estimation of tumor dissemination at diagnosis. It is well proved that disseminated tumor cells in the bone marrow are the most important prognostic factor in operable breast cancer [1]. Its prognostic role become visible at late periods after surgery (sometimes, 25 years). Taking in mind that women with breast cancer are around 60-year-old, one may see that the relapses arises usually after 80. It appears in brain, lung and other distant organs as hematologic metastases, and unfortunately, nothing is possible to do in the most of those cases, so the main reasons of deaths in breast cancer are distant metastases. Hematological dissemination of breast cancer takes place at very early stages of the diseases, sometimes even at carcinoma in situ [2]. Postmenopausal osteoporosis is a silent disease with not much of symptoms until fractures occur. It is characterized by low bone mineral density (BMD) and changes in bone microarchitecture that reduces bone strength and there is increase in risk of fractures [1]. After menopause, estrogen deficiency is the major contributing factor. Because of this there is an increase in RANK-ligand (RANKL) and a decrease in osteoprotegerin (OPG) secretion from osteoblasts. This imbalance induces fast bone loss, and thus there is increase in risk of fractures. WHO has identified osteoporosis as a major public health concern [2]. In 2002, a hysteroscopic permanent sterilization option was made available: The Essure® System. It consists of a set of 2 coil designated to induce fallopian tube fibrosis and subsequent tubal occlusion. Three months after placement, the tubal occlusion is confirmed with a hysterosalpingogram which will then ensuretubal blockage [2]. The coils could be inserted in an office setting with no anesthesia or only minor sedation, providing a safe alternative avoiding the need for surgical intervention. The disease, its diagnosis and treatment impose considerable negative physical and psychosocial effects on the patients and severely threaten their quality of life. Surgical treatment with different types is the most used option for breast cancer treatment. In this mini-review we briefly reviewed the effects of different surgery types on the breast cancer patients’ quality of life. This does not exist only in lower and middle class, but highly educated women also suffer because of male dominant society. It might be physical, sexual, or psychological violence. Our Pakistani culture, tradition, norms and values support this act. This is deeply believed in our society that women are inferior to men.
