A Study on Sildenafil Citrate in the Treatment of IUGR & Oligohydramnios

Image
Intra Uterine Growth Restriction is a challenge to clinicians all over the world because no definitive treatment is available, and many empirical therapies are tried for treating the same. The present study was a multi-centric hospital based prospective observational study conducted in three tertiary care centers in Northern India on 200 patients with late onset IUGR and oligohydramnios. Sildenafil citrate 25mg 8 hourly was given till delivery. All the outcomes were noted and analyzed. After sildenafil therapy, 87 out 0f 90 patients with only IUGR, 35 out of 40 cases of oligohydramnios only and 64 out of 70 patients with IUGR & oligohydramnios both had marked improvement & 14 cases showed no improvement. From this study we concluded that adding sildenafil citrate in the treatment of IUGR and oligohydramnios can be a beneficial option. DIC-A Nightmare in Obstetrics

Image
Abstract Disseminated Intravascular Coagulation (DIC), the most catastrophic obstetric acute emergency, is characterized by an inappropriate activation of coagulation & fibrinolytic system triggered by conditions like abruptio placentae & amniotic fluid embolism (release of thromboplastin intravascularly) or by diffuse endothelial damage as a result of pre-eclampsia. Clinical manifestation of severe bleeding and damaged coagulation profile are hallmarks of diagnosis, although histopathological demonstration of fibrin deposits is the pathognomonic feature of DIC. Understanding the basic pathophysiology, tackling the underlying etiological factor and replacement of the lost blood and specific components cover the principles of management of this condition. Keywords: DIC; FFP; Thromboplastin coagulation failure; Coagulopathy Abbreviations : DIC: Disseminated Intravascular Coagulation; HELLP: Hemolysis Elevated Liver Enzymes and Low Platelets Syndrome; IUFD: Intrauterine Fetal Dea...

A Critical Review on Postpartum Depression and Physical Activity

Image
One of the serious complications that are being experienced by women over many eras has been the issue of postpartum syndrome and the ways of coping with it. It is clear that many women can lose attention to their lives, their babies and their husbands. Actually, they become less happy and more stressful. Yet, this is not bound to the physicality of their beings, it is because of hormones. As is it is known that after childbirth, a dramatic drop in hormones (estrogen and progesterone) in women’s body may contribute to postpartum depression. Other hormones produced by mothers’ thyroid gland also may drop sharply-which can leave mother feeling tired, sluggish and depressed. Frankly, women need to feel emotionally supported in those hard times. Yet how can it be possible when they are fully confronted with the degenerated body image, inconvenient hormone rates and stressed mind after birth? Possibly they can be backed up by their husbands or other family members and they can get proper mo...

Integrating CERVICO-Vaginal Smear into the Overall Management of Women Living with Human Immunodeficiency Virus (HIV) at the Day Hospital of the Infectious Diseases Department at the University Hospital Mohammed VI in Marrakesh

Cervical cancer is the fourth leading cause of cancer and cancer death among women worldwide [1]. According to the International Agency for Research on Cancer (IARC) in 2018, the global incidence is estimated at 570,000 new cases per year, of which more than 80% occur in developing countries [1]. In Morocco, the cervical cancer represents a major public health problem. It ranks second, after breast cancer in Moroccan women [2]. The incidence of cervical cancer remains high among women living with HIV. However, many studies have shown that this population has an increased risk of developing intraepithelial cervical neoplasia [3]. In the fight against cervical cancer, Morocco has a national cancer control programme under the aegis of the LALLA SALMA Foundation since 2006, thus enabling the creation of reference centers for screening and diagnosis of cervical cancer [4]. Women living with HIV often escape this program because of fear of stigmatization and disclosure of their HIV status. I...

Clinical Importance of Pain Sensitization in Gynecology

Image
Peripheral and central sensitizations are conditions of the nervous system that are characterized by increased tone and responsiveness, a lower threshold for the experience of pain and a tendency for pain to persist long after the original stimulus has ended [1]. The mechanism of the development of chronicity is under intense investigation involving a variety of important biochemical and sensitization processes. From a clinical perspective however, the situation can be summed up that pain per se, if not kept under control, may initiate the processes causing the transition from acute to chronic pain. Basic Investigations Investigations into the origins of central sensitization in particular, have focused on the role of continuous peripheral drive from the nociceptors (defined as neurons that are activated by noxious stimulation). It has been shown these nociceptors can become sensitized after being injured and develop a lower threshold for activation. Further, the persistence of affer...

Image
Sociology appraising various dimensions of human life, will have a close look at the dependency of women on gynecology at current times. Many factors such as longer life expectancy, late marriage, urbanization, industrialization, curiosity for further rejuvenation and many more, have contributed to further tendency toward gynecology by women in modern time. Previously, and when science of gynecology had not advanced, women would easily die in young age. Women are exposed to more illnesses and health disorders as compared to men. Women at any age are anxious to keep on their beauty and body shape. Such a value highly attaches them to gynecologists. Modern gynecology originating in Western countries, gradually entered the developing countries with special reference to the richer ones. In modern time, gynecology has become a sort of tourism (Medical Tourism), in that, the well to do women go to other countries for special treatments. Physical and Gynecological Outcomes among South African Women Survivors of Rape

Image
Introduction: South Africa has one of the highest rates of sexual violence against females in the world [1,2]. Residual outcomes among rape survivors in South Africa are complex, and include symptoms of depression, and post-traumatic stress (PTSS) [3,4] as well as physical and gynecological problems [5,6]. While South African rape survivors report physical and mental health symptoms [7]. We know little about the internal (i.e. self-management) and external (i.e. support from others) coping strategies that they use over time.   Objectives: This study conducted secondary analyses of South African female rape survivors who reported experiences of rape within six months of being interviewed and compared their physical and mental health, coping strategies, rape myths and social undermining of friends and families one year later. The goal is to foster a discussion of post-rape outcomes that may be utilized to develop recovery interventions for South African women rape survivors who may...
